1. Call to schedule an appointment at 480-963-8833. If you have insurance, please have the information ready.

2. Fill out paperwork ahead of time. Click here for  Patient History Form
      - We can also fax or mail you the form in advance upon requests.

3. Arrive 15 minutes before your appointment. This will allow time for us to process your paperwork and make sure you are seen as soon as possible.
      - If it is your child’s exam, this extra time will let him acclimate to the office. We have toys in the waiting room.

4. Plan around 1 hour for the visit and longer if you plan on picking out glasses
      - Comprehensive exams, sensory motor exams, and contact lens exams are all scheduled for 45min to an hour.
      This gives you plenty of time with your doctor to discuss all matters in depth.
